At Skin Care Specialty Physicians we treat your right to privacy with the utmost care. We will only discuss your medical information with yourself or those people authorized in our system. If you would like to add someone, feel free to give us a call or fill out our HIPPA Form in the packet above and bring it with you to your appointment.
Skin Surgery Patients
If you are scheduled to undergo surgery at our Mohs & Skin Surgery Center with Dr. Saif Syed, Dr. Zain Syed, or Dr. Amita Bagal, please complete the forms in the packet below prior to your appointment and bring them with you on the day of your surgery.
