Skin Surgery Patients
If you are scheduled to undergo surgery at our Mohs & Skin Surgery Center with Dr. Saif Syed, Dr. Zain Syed, or Dr. Amita Bagal, please complete the forms in the packet below prior to your appointment and bring them with you on the day of your surgery.
